Flex-HRP is a healthcare reward point program that enables card holder  to earn or purchase points at participating retail merchants or at www.carepointscard.com and be able to redeem those points to pay for healthcare services at participating healthcare providers with up to 40% in savings as reward from the participating healthcare providers in addition to the following benefits:

1. Personalized Shopping Network “PSN”

• Ability to create a Personalized Shopping Network “PSN” from a list of nationally recognized retail merchants that offer reward program and or discounted program where those reward points and or cash rebates will be converted to Flexible-Healthcare Reward Points (Flex-HRP) program. The type of network includes Gas stations, Healthcare providers, Grocery stores and department stores that offer rewards, rebates and or discounted programs.

2. Flexible-Healthcare Reward Points (Flex-HRP) program offer:

• Non major medical revolving CareCredit-Points (CC-P) up to 1,000 CC-P or $1,000.00 which are to be used for non major medical expenses at participating healthcare providers that accept CarePoints Card with up to twelve (12) months to earn more reward points to repay the CC-P. payroll deduction
• Major medical insurance deductable offers up to 5,000 CC-P or $5,000.00 to meet insured card member’s major medical deductable with up to 24 months repayment terms with earned rewards from PSN and Care-Points earned from healthcare providers and insurance companies
  
• Member gains access to the use of their CC-P within the members Flex-HRP program with the use of CarePoints Card at participating healthcare providers that accept the card.
• Ability to view account balance via CarePoints Card website
• Ability to purchase CC-P online and at participating PSN into the Flex-HRP program
• Earn triple reward points on every net dollar spent at participating healthcare providers
• Ability to convert  earned reward or rebate program to Flex-HRP
• The Flex-HRP earned rewards are valid for healthcare expenses with no restriction on the use of fund for any healthcare expenses within participating healthcare providers that accept the card and your Flex-HRP will not expire.
• When you leave the program your Flex-HRP less any charge shall be returned to you by check within 30 business days of cancelation of your membership.

3. Eligibility:

• Eligibility for Flex-HRP Platinum Card requires a member to earn at least 250 CC-P per quarter to be eligible for the Flex-HRP benefits
• Eligibility for Flex-HRP major medical deductable benefit requires that the member maintain the minimum quarterly earned 250 CC-P and maintain a major health insurance policy that requires at least $2,500 deductable and remain active for at least three consecutive months prior to receiving the benefit.
• The eligible amount is subject to CarePoints Card evaluation of member’s repayment capability notwithstanding everyone qualifies for a Flex-HRP up to 50% more than the CC-P cash value on their card account.
