Eligibility for Healthcare Provider Membership in CarePoints Merchant Program

• You must have been in business for two years and be duly licensed to practice within the state where your business is located
• You must commit to Sponsor a minimum of 250 activated CarePoints Card members within a given year
• You must set and publish your practice fee schedule to be published in the CarePoints Merchant section of CarePoints Card website
• You must display CarePoints program promotional material within your business premises
• You must have an existing Credit Card Merchant Account to be upgraded to accept CarePoints Card
